Multiple console failure during the same case????

As a topic of discussion, has anyone ever had a VAD console failure in both their primary and backup devices during the same case? Were the failures different or the same? Did you implament new policies to guard against this incidence? Was the failure due to human error or unforeseeable mechanical failure?
